- uniform order - due 6/20/09 - see below

- medical release (downloadable form below)
- birth cert copy
- 1x1 inch photo
- check for fall fees, payable to WASA Eagles FC ... notes: 1) uniform $ will be due in early
August, and 2) please notify your team admin if you have requested scholarship funds from Jeff Dybdahl (contact Jeff
at dybby@juno.com for all scholarship requests)
